Treatment of hemangiomas of infancy with imiquimod cream
Imiquimod is an immunomodulator that is currently approved for the treatment of genital warts, actinic keratoses, and superficial basal cell carcinoma. Catherine McCuaig, MD, presented preliminary data from a study that she and her colleagues at Hôpital Ste-Justine in Montreal, Canada, are performing to evaluate the efficacy of imiquimod 5% cream in the treatment of hemangiomas of infancy. The mechanism of action of this medication makes it an interesting candidate for the treatment of this condition. Imiquimod leads to the secretion by T cells of multiple antiangiogenic cytokines, including interferon-alpha (IFN-alpha), and systemic IFN-alpha is an established therapy for hemangiomas.
A few case reports in the literature have reported the successful treatment of hemangiomas with imiquimod, including a report of a 4-month-old infant whose hemangioma completely resolved after 10 weeks, and a report of 10 patients treated for up to 16 weeks, 70% of whom achieved complete or excellent response. When reading such reports, it is important to consider the patients' ages and the duration of therapy, because hemangiomas undergo spontaneous involution, usually beginning around 9 to 12 months of age.
The study at Hôpital Ste-Justine will be the first prospective study evaluating imiquimod in the treatment of hemangiomas of infancy. Preliminary data suggest that there is a diminution in volume as well as erythema with application of imiquimod cream 3 to 7 times per week for 16 weeks.

i lavori sono noti ma soprattutto i primi due hanno dei grossi limiti.

Carissima Mirella,
come si evince dai dati bibliografici l'utilizzo dell'Imiquimod negli ultimi tre anni è stato sperimentato in pochi casi con risultati discutibili.
La crema in oggetto favorisce la produzione endogena di interferon alfa che nel neonato potrebbe avere effetti collaterali importanti e inoltre è difficile da modulare e dosare.
La ditta produttrice 3M non ha mai sottolineato questa indicazione. 
L'Imiquimod resta una terapia valida per l'epitelioma basocellulare superficiale e per i condilomi recidivanti.
